Growth meets legal uncertainty

  • Third-quarter revenue rose 22 percent as da Vinci and Ion procedures increased 15 percent combined.
  • A proposed 450 million US-dollar antitrust settlement reduced GAAP earnings but requires court approval.
  • Management expects 2026 da Vinci procedure growth near the midpoint of its 14 to 15 percent range.

Intuitive Surgical reported strong operating growth for the third quarter ended September 30, 2026, but a proposed antitrust settlement weighed heavily on reported earnings. The company disclosed the results and legal agreement on October 20 in a SEC filing.

Procedure growth stays broad

Revenue increased 22 percent to 3.05 billion US dollars from 2.51 billion US dollars a year earlier. The main recurring revenue stream, instruments and accessories, grew 18 percent to 1.80 billion US dollars. Systems revenue rose to 769 million US dollars from 590 million US dollars as placements increased.

Worldwide procedures across the da Vinci and Ion platforms grew approximately 15 percent year over year. Da Vinci procedures increased approximately 14 percent, while Ion procedures advanced approximately 31 percent. Intuitive placed 546 da Vinci systems, compared with 427 in the prior-year quarter, including 315 da Vinci 5 systems against 240 a year earlier.

The installed base also expanded. Da Vinci systems reached 12,094 at September 30, up 12 percent from 10,763 a year earlier. The Ion installed base rose 21 percent to 1,152 systems. Intuitive said customers performed more than 10,000 procedures per day on average during the quarter.

Settlement cuts GAAP earnings

GAAP net income attributable to Intuitive Surgical fell to 523 million US dollars, or 1.49 US dollars per diluted share, from 704 million US dollars, or 1.95 US dollars per share, in the third quarter of 2025. The result included a 450 million US-dollar pre-tax charge tied to a proposed settlement of previously disclosed antitrust class-action litigation.

The agreement covers claims concerning servicing of da Vinci systems and EndoWrist instruments. Intuitive said it made no admission of liability or wrongdoing. The settlement remains subject to preliminary and final court approval, so its final legal and financial status is not yet settled.

On a non-GAAP basis, which excludes the litigation charge and other specified items, net income rose to 1.05 billion US dollars, or 3.00 US dollars per diluted share, from 867 million US dollars, or 2.40 US dollars per share. The contrast highlights the difference between the company's underlying operating momentum and the one-time legal burden.

Outlook remains unchanged

Management kept its 2026 outlook for worldwide da Vinci procedure growth of approximately 14 to 15 percent and expects growth near the midpoint of that range. It also projects a non-GAAP gross margin of 68.7 to 69.3 percent and operating expense growth of 11 to 13 percent for the year.

Dave Rosa, Intuitive's chief executive, said adoption remained broad across the multi-port, single-port and Ion platforms. For investors, the key issue is whether procedure growth and the expanding installed base can continue to offset legal costs, tariffs and the higher spending required to support expansion.

The real-time quote of Intuitive Surgical stock traded at 430.75 euros on Lang & Schwarz at 14:32 CET on Thursday, November 5.
