Intuitive Surgical’s AI Vision Can’t Break the Stock’s Downward Spiral

Strong Quarter, Cautious Guidance
Intuitive Surgical’s second-quarter results, released weeks ago, were anything but weak. Revenue climbed 18.5% to $2.89 billion, while adjusted earnings per share came in at $2.80 — both comfortably ahead of analyst estimates. The installed base of da Vinci systems expanded 12% year-over-year to 11,710 units, with 246 of the new da Vinci 5 platforms shipped in the quarter alone.
The rub came from management’s forward view. The board declined to raise its full-year procedure-growth forecast, keeping it at 13.5% to 15.5% for 2026. The culprit: the expiration of enhanced tax credits under the Affordable Care Act, which the company described as having a “moderately negative effect” on elective procedures during the quarter. Rising costs for semiconductor components and freight rates are also squeezing near-term margin expectations, even as the underlying business performs well.

CEO Dave Rosa said the AI is designed to shorten surgeons’ learning curves and improve care quality. The presentation culminated in a live telesurgery demonstration connecting Sunnyvale and Atlanta.
The same day, the company also received FDA clearance for its AirSeal system. But none of that technological momentum translated into share-price support.
A Rival’s FDA Nod Adds to the Gloom
Compounding the stock’s woes, the FDA on July 23 also cleared Johnson & Johnson’s Ottava Robotic Surgical System for multiple soft-tissue procedures, including gastric bypass and gallbladder removal. J&J plans an initial commercial launch with select U.S. customers. While J.P. Morgan analysts still view da Vinci as the preferred system in the robotic surgery market, the entry of a healthcare heavyweight adds a new layer of uncertainty.
Momentis Surgical also secured FDA clearance the same day for the multiport configuration of its Anovo robot, which combines natural-orifice, single-port, and multiport capabilities on one platform. The competitive field is visibly thickening, even if Intuitive Surgical’s installed base and brand remain dominant — as evidenced by the recent deployment of a da Vinci Xi system for thoracic surgery at Surrey Memorial Hospital in Canada.
