Siemens Healthineers has given shareholders a first look at the year ahead, and the headline number cuts both ways. The medical-technology group is guiding for comparable revenue growth of 4–5% in fiscal 2027, paired with adjusted unadjusted earnings per share of €2.20–2.40. The top of that profit band sits well under the €2.42 the company has just booked for fiscal 2026 — a fact that framed much of the market's reaction.

The 2026 base itself was solid enough. Comparable revenue rose 4.0% over the year, and the final quarter was the strongest stretch, with sales up 5.9% on a like-for-like basis and an adjusted EBIT margin of 15.9%. Free cash flow for the three months came in at €703 million.

Where the Guidance Falls Short

Consensus had been looking for more. As Reuters reported, analysts were modelling 5.5% revenue growth for 2027, and their EPS expectation of €2.37 sits comfortably inside the range management has now published — meaning the profit guide is not the sticking point so much as the pace of growth. Two drags were singled out: continued weakness in China and a shrinking diagnostics business, whose comparable sales fell 4.2% in the fourth quarter of fiscal 2026. The stock came under selling pressure on Thursday in the wake of the annual figures and the outlook.

Should investors sell immediately? Or is it worth buying Siemens Healthineers?

Two Houses, Two Verdicts

The sell-side split cleanly the following day. Barclays lifted its target to €51 from €47.50 while reiterating an Overweight rating, with analyst Hassan Al-Wakeel pointing to an expected strong product cycle in 2027 as the basis for his constructive stance. Deutsche Bank Research, by contrast, kept its Hold rating and €39 target unchanged after digesting the results — a spread of €12 between the two houses on where the shares should trade.

Board Reshuffle and a Breakup on the Ballot

Alongside the numbers, Healthineers set out a governance shake-up. The supervisory board will ask the 2027 annual general meeting to appoint Melanie Kreis, Heyo Kroemer, Jonathan Lewin and Pascale Witz as new shareholder representatives. Roland Busch is stepping down early, and three further seats expire at that same meeting.

The bigger structural question — the planned separation — will also go to a vote, with both the Healthineers and Siemens shareholder meetings scheduled for February 2027 to decide on the carve-out. Investors get their next formal touchpoint on 9 November 2026, when a roadshow with JPMorgan is held in London.
